Family Shelter Survey

Participation is voluntary and anonymous. The questionnaire does not ask for names, contact information, or street addresses. Please do not include information that could identify you or anyone else in the open-text response. Individual questions may be skipped.

Responses are stored in a private file outside the public website folder on the hosting server. They are meant for staff review to understand community perspectives, not to provide shelter intake or individual support. No third-party analytics are installed on the survey.

Contact inquiries

If you use the contact form, we store the name, email address, organization (if provided), and message you submit. These are meant for staff follow-up. Messages do not trigger automatic email notifications and should be reviewed by staff through the hosting account.

Data management

Website administrators should restrict access to survey and contact files, download and delete unnecessary records according to their organization's retention policy, and make sure the hosting environment is configured securely. This page should be reviewed by HCT before public launch.
